About the Role
This is not your typical in-office Medical Assistant or LPN position. Instead of assisting during exams or preparing labs, you will play a vital role in helping patients navigate their healthcare journey. As a Patient Care Coordinator, you will focus on Chronic Care Management (CCM), Behavioral Health Integration (BHI), Principal Care Management (PCM), and Remote Physiological Monitoring (RPM). Your role will involve working closely with patients through monthly phone calls, identifying gaps in care, and ensuring they receive the right resources and support.
What You'll Do
Support and guide patients through their healthcare journey via phone conversations.
Develop and maintain relationships with patients to ensure their needs are met and they feel heard.
Complete individualized patient care plans and coordinate care using a dedicated software platform.
Document interactions accurately with patients, caregivers, and healthcare providers.
Communicate effectively with medical staff to ensure comprehensive care plans are in place.
Stay up-to-date with healthcare best practices and adapt to new processes and software as needed.
Provide excellent customer service while maintaining a positive and professional demeanor.
What We're Looking For
Bilingual in English and Spanish (required).
Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), or Nationally Certified Pharmacy Technician (CPhT).
At least 2 years of experience in a healthcare-related role.
Strong knowledge of healthcare terminology and care management principles.
Experience with Electronic Medical Records (EMR) systems.
Excellent communication skills and the ability to problem-solve effectively.
Ability to work independently in a fast-paced environment while maintaining strong attention to detail.
Proficiency in computer skills, including navigating multiple systems.
Preferred Qualifications
Experience with CMS Chronic Care Management (CCM) guidelines.
Background in care management for chronic conditions.
Previous experience in remote patient support or telephonic care coordination.

Work Schedule
Full-time position (40 hours per week)
